Help With Your Initial VA Claim

You must use official VA forms to file your claim.

Use VA Form 21-526EZ (print or online) to claim any disabilities you have today that are related to your time in service.

Your military medical records and any federal records identified in your claim, such as records from the Social Security Administration, will be obtained by the VA.

  1. VA Form 21-526EZ.

  2. Private medical records OR completed VA Form 21-4142 for VA to obtain your records.

  3. Supportive VA Forms if they apply to your claims.

  4. Optional: Nexus statements, lay/buddy statements or DBQs if they apply.

Filing Your Initial Application
